Qsymia

Phentermine/Topiramate, sold under the brand name Qsymia, is a prescription medication used for chronic weight management in adults and certain adolescents, in conjunction with a reduced-calorie diet and exercise. 

Key Components:

  • Phentermine: A stimulant and anorectic (appetite suppressant).
  • Topiramate: An anticonvulsant that can also reduce appetite and enhance satiety (feeling full). 

How it Works:

  • Appetite Suppression: Phentermine works in the brain to decrease appetite.
  • Increased Satiety: Topiramate is believed to contribute to weight loss by reducing appetite and increasing the feeling of fullness. 

Effectiveness:

  • Clinical trials have shown that the combination of phentermine and topiramate can lead to significant weight loss compared to diet and exercise alone.
  • Patients taking the highest dose of phentermine/topiramate have been shown to lose an average of 9-11% of their body weight, and those taking lower doses lose an average of 7-8%. 

Safety and Side Effects:

  • Common Side Effects: Dry mouth, constipation, paresthesia (numbness or tingling), insomnia, and altered taste.
  • Serious Risks: Phentermine/topiramate is a controlled substance due to the potential for misuse and dependence. Other serious risks include birth defects (cleft lip/cleft palate), increased heart rate, suicidal thoughts or behaviors, vision problems, and metabolic acidosis.
  • REMS Program: Due to the risk of birth defects, Qsymia is only available through a special program (Risk Evaluation and Mitigation Strategy) requiring certified pharmacies and specific patient monitoring. 

Important Considerations:

  • Contraindications: Phentermine/topiramate should not be taken by pregnant women, individuals with glaucoma, hyperthyroidism, or those taking certain MAO inhibitors.
  • Interactions: It’s important to discuss all medications and supplements with your doctor, as phentermine/topiramate can interact with other drugs, potentially impacting their effectiveness or increasing the risk of side effects.
